Naltrexone is a medication from the class of opioid antagonists. It works by blocking specific receptors in the brain where the body’s own endorphins normally dock. These endorphins are a key reason why alcohol feels good. When naltrexone occupies those docking sites, the reward signal can’t get through. You can still drink, but the warm glow and the sense of relief that alcohol usually delivers are dulled or absent.
Naltrexone is one of only three medications currently approved by the U.S. Food and Drug Administration (FDA) for the treatment of Alcohol Use Disorder, alongside acamprosate and disulfiram. It is available as an oral tablet (sold under the brand name ReVia, among others) and as a once-monthly extended-release injection marketed as Vivitrol. How Naltrexone Works in the Brain #
To understand the mechanism, it helps to look at what normally happens when a person drinks. Alcohol triggers the release of endorphins in the brain. These endogenous opioids bind to μ-opioid receptors and produce feelings of pleasure and relaxation. At the same time, this pathway stimulates the release of dopamine in the nucleus accumbens, the core hub of the brain’s reward system. That dopamine surge is what teaches the brain to file drinking away as something worth repeating. Over time, this process builds an addiction memory.
Naltrexone steps into this chain by occupying the μ-opioid receptors without activating them. The endorphins are still released when alcohol is consumed, but they find no open receptors to bind to. As a result, the usual reward cascade is interrupted: less dopamine release, less pleasure, less drive to keep drinking. In plain terms, the alcohol still tastes the same, but it no longer “does anything” in the brain.
This plays out on two levels. In the short term, it can reduce craving because the expected payoff doesn’t materialize. Over the longer term, repeated experiences of drinking without reward can weaken the learned association between alcohol and pleasure. Researchers call this pharmacological extinction, essentially a medication-assisted unlearning of the reward expectation.
How Naltrexone Is Taken #
Naltrexone comes in two forms:
Oral tablets. The standard dose is 50 mg once daily. Brand names include ReVia (and previously Depade, which has been discontinued). Some clinicians start patients at 25 mg for the first week to minimize side effects, particularly nausea. The tablets are usually taken with food.
Extended-release injection (Vivitrol). Vivitrol delivers 380 mg of naltrexone as a deep intramuscular injection into the gluteal muscle, releasing the drug steadily over about four weeks. This form eliminates the need for daily pill-taking and is especially useful for people who struggle with medication adherence. Vivitrol requires a prescription and is administered by a healthcare provider. Because it bypasses the digestive system, it also avoids most of the gastrointestinal side effects that oral naltrexone can cause.
An important clinical requirement applies to both forms: a person must be fully detoxified from opioids before starting naltrexone. The general recommendation is at least 7 to 10 days opioid-free before the first dose. Starting naltrexone while opioids are still in the system can trigger precipitated withdrawal, a sudden and severe withdrawal reaction.
The Sinclair Method (TSM) #
One of the most discussed alternative approaches to naltrexone use is the Sinclair Method, named after Finnish-American researcher John David Sinclair. It differs fundamentally from the standard prescribing approach. Instead of taking naltrexone every day while maintaining abstinence, the Sinclair Method calls for taking the medication only when drinking, about one hour before the first drink.
The logic is rooted in classical conditioning. If alcohol is repeatedly consumed while naltrexone blocks the reward, the brain gradually unlearns the connection between drinking and pleasure. Sinclair called this pharmacological extinction. On days when no drinking occurs, the medication is intentionally skipped so that the brain’s natural endorphin responses to other activities (exercise, social connection, food) remain intact.
The Sinclair Method has a sizable following in the United States and in online recovery communities. Claudia Christian’s book and the documentary “One Little Pill” brought it to a broader public audience, and several telehealth providers now specialize in prescribing naltrexone specifically for TSM. Clinical studies, particularly from Finland, have shown promising results for reducing total alcohol consumption.
That said, the Sinclair Method remains controversial in mainstream addiction medicine. The primary concern is that it assumes continued drinking as part of the treatment, which conflicts with abstinence-based approaches. The American Society of Addiction Medicine (ASAM) and most U.S. treatment guidelines recommend naltrexone as support for abstinence, not as a companion to ongoing consumption. People considering TSM should discuss it with a physician who understands both approaches.
Naltrexone vs. Naloxone – What’s the Difference? #
The names sound almost identical, and both are opioid antagonists. But they serve very different purposes. Naltrexone is taken orally or injected as a depot and works for hours to days. It is prescribed for relapse prevention in alcohol and opioid use disorders.
Naloxone, by contrast, is an emergency medication used to reverse opioid overdoses. It is typically administered intravenously, intramuscularly, or as a nasal spray (Narcan) and takes effect within minutes. Its duration of action is much shorter than naltrexone’s. Naloxone plays no role in alcohol treatment. It does, however, appear as a combination partner in certain opioid medications (for instance in Suboxone, paired with buprenorphine) to deter misuse.
Side Effects #
Naltrexone is generally well tolerated. The most common side effects involve the gastrointestinal tract: nausea (especially during the first few days), stomach cramps, reduced appetite, and occasional diarrhea. Some people also report headaches, fatigue, dizziness, sleep difficulties, or a sense of emotional flatness.
That last point deserves a closer look. Because naltrexone blocks opioid receptors, it can dampen the body’s natural endorphin responses across the board, not just those triggered by alcohol. Some people notice that everyday pleasures like a good meal, a hard workout, or a social gathering feel slightly muted. This effect varies widely from person to person and tends to be most noticeable in the first weeks of treatment. It stands in a certain tension with the natural recovery of the reward system through abstinence and neuroplasticity.
Because naltrexone is metabolized by the liver, it can be hepatotoxic at higher doses. The FDA labeling includes a black box warning about the risk of liver damage. Liver function tests are recommended before starting treatment and periodically thereafter.
When Naltrexone Must Not Be Used #
Naltrexone is contraindicated in anyone currently taking opioids, whether prescription painkillers, methadone or buprenorphine maintenance therapy, or illicit opioids. Naltrexone would abruptly block the opioid effect and precipitate acute withdrawal, which can be life-threatening.
Other contraindications include severe liver disease (such as advanced liver cirrhosis or acute hepatitis) and known hypersensitivity to the drug. Naltrexone is also not recommended during pregnancy or breastfeeding due to insufficient safety data.
A practical point that is sometimes overlooked: anyone on naltrexone who requires emergency pain management poses a clinical challenge, because standard opioid painkillers will not work while the receptors are blocked. Patients are advised to carry a medical alert card or wear medical identification indicating naltrexone use.
Where Naltrexone Fits in Alcohol Treatment #
The landmark COMBINE study (2006), one of the largest clinical trials on alcohol treatment, found that naltrexone combined with medical management was effective in reducing heavy drinking days. Results were strongest when the medication was paired with at least some form of professional support.
Naltrexone works best as one component of a broader treatment plan. It is not a cure for Alcohol Use Disorder on its own. The strongest outcomes in clinical research appear when naltrexone is combined with psychotherapy, whether cognitive behavioral therapy, motivational interviewing, or a structured relapse prevention program.
Compared to acamprosate, which stabilizes the brain’s glutamate system and primarily helps manage the inner restlessness and anxiety after quitting, naltrexone targets the reward pathway. The two medications address different aspects of the problem and may work better for different individuals.
